Overview

This is a three-drug chemotherapy regimen consisting of carboplatin, irinotecan, and paclitaxel. All three are cytotoxic agents with distinct mechanisms of action. Carboplatin is an organoplatinum alkylating agent that forms DNA crosslinks and inhibits DNA synthesis. Irinotecan is a topoisomerase I inhibitor that prevents DNA unwinding and replication by stabilizing the cleavable complex between topoisomerase I and DNA. Paclitaxel is a microtubule-stabilizing agent that disrupts mitosis by promoting tubulin polymerization and inhibiting microtubule disassembly. This combination has been studied primarily in advanced non-small cell lung cancer (NSCLC), where it has demonstrated antitumor activity but also significant hematologic toxicity[1][2]. The regimen is typically administered intravenously in 21-day cycles.
