This combination therapy consists of ivonescimab (a bispecific antibody targeting PD-1 and VEGF-A) plus the chemotherapy agents pemetrexed and carboplatin. The regimen works through multiple mechanisms: ivonescimab simultaneously blocks PD-1/PD-L1 interaction (relieving immunosuppression) and VEGF-A/VEGFR2 binding (inhibiting tumor angiogenesis), while pemetrexed inhibits multiple folate-dependent enzymes involved in DNA synthesis and folate metabolism, and carboplatin forms DNA adducts that disrupt cellular replication. This combination received approval in China in May 2024 for EGFR-mutated locally advanced or metastatic non-squamous NSCLC in patients who have progressed after tyrosine kinase inhibitor therapy.
