Overview

This is a combination regimen consisting of three agents—carboplatin, ixabepilone, and trastuzumab—used primarily in the treatment of HER2-positive breast cancer, including both metastatic and early-stage disease. - **Carboplatin** is a platinum-based chemotherapeutic agent that induces DNA crosslinking and apoptosis in rapidly dividing cells. - **Ixabepilone** is a synthetic analog of epothilone B that stabilizes microtubules by binding to β-tubulin, thereby inhibiting cell division; it is particularly effective in taxane-refractory cancers. - **Trastuzumab** is a monoclonal antibody targeting the human epidermal growth factor receptor 2 (HER2), blocking downstream signaling pathways involved in cell proliferation and survival. The combination has been evaluated as first-line therapy for HER2-positive metastatic breast cancer (MBC) as well as neoadjuvant therapy for early-stage HER2-positive breast cancer. Clinical studies have shown response rates around 44% with an acceptable toxicity profile; common grade 3–4 adverse events include neutropenia, thrombocytopenia, fatigue, nausea, diarrhea, and neuropathy[1][6]. The regimen may be considered an alternative to standard docetaxel-containing combinations for patients who are refractory or intolerant to taxanes[5][6].
