Carboplatin + melphalan is a combination chemotherapy regimen consisting of two alkylating agents, carboplatin and melphalan. Both drugs are small molecule chemotherapeutics that induce DNA crosslinking, leading to inhibition of DNA replication and cell death. This combination is used primarily in high-dose regimens for the treatment of advanced stage or chemoresistant solid tumors, often followed by autologous stem cell rescue (bone marrow transplantation). The rationale for combining these agents lies in their complementary toxicity profiles and synergistic antitumor effects. Carboplatin dosing may be adjusted based on glomerular filtration rate (GFR) to optimize efficacy while minimizing renal toxicity. The regimen is associated with severe but generally tolerable hematological and gastrointestinal toxicities[1].
