This combination chemotherapy regimen was evaluated by the SWOG Cancer Research Network for the treatment of locally advanced or recurrent urothelial malignancies, including bladder and urethral cancers. The protocol consists of a neoadjuvant phase involving carboplatin, paclitaxel, and gemcitabine, followed by a consolidation phase of concurrent cisplatin and radiation therapy. Gemcitabine is a nucleoside analog that inhibits DNA synthesis by targeting ribonucleotide reductase and DNA polymerase. Carboplatin and cisplatin are platinum-based alkylating agents that induce DNA damage through cross-linking, while paclitaxel is a taxane that stabilizes microtubules to prevent cell division. The regimen aims to maximize tumor cell death by combining multiple cytotoxic mechanisms with radiation therapy.
