This drug is a combination chemotherapy regimen consisting of three agents: - **Carboplatin** is an organoplatinum alkylating agent that causes DNA crosslinking, leading to apoptosis of rapidly dividing cancer cells. It is primarily used in the treatment of various cancers including ovarian and non-small cell lung cancer (NSCLC). - **Paclitaxel** is a microtubule inhibitor that stabilizes microtubules and prevents their disassembly, thereby inhibiting mitosis and inducing apoptosis in cancer cells. - **Thalidomide** has immunomodulatory, anti-inflammatory, and anti-angiogenic properties. It modulates the immune system by affecting cytokine production and inhibits angiogenesis which can reduce tumor blood supply. The combination has been studied particularly in unresectable stage III NSCLC patients receiving chemoradiotherapy. Clinical trials showed that adding thalidomide to paclitaxel plus carboplatin with thoracic radiation increased toxicities such as thromboembolism but did not improve overall survival or progression-free survival compared to chemotherapy-radiotherapy alone[1][5]. Thalidomide dosing started at 200 mg daily with escalation up to 1000 mg based on tolerability.
