This is an investigational combination therapy consisting of three agents: carboplatin, paclitaxel, and vandetanib. Carboplatin is an alkylating platinum-based chemotherapeutic that induces DNA crosslinking and apoptosis in rapidly dividing cells. Paclitaxel is a microtubule-stabilizing chemotherapeutic that inhibits cell division by preventing microtubule depolymerization. Vandetanib is an oral small molecule tyrosine kinase inhibitor targeting vascular endothelial growth factor receptor 2 (VEGFR2), epidermal growth factor receptor (EGFR), and RET proto-oncogene, thereby inhibiting tumor angiogenesis and proliferation. The triple regimen has been studied in phase II clinical trials for non-small-cell lung cancer and in early-phase studies for esophageal cancer[1][2][6]. The rationale behind this combination is to enhance antitumor efficacy by combining cytotoxic chemotherapy with targeted inhibition of key signaling pathways involved in tumor growth and angiogenesis.
