CB-182,804 is a novel, next-generation polymyxin B analogue developed by Curza to address the critical need for new treatments against multi-drug resistant (MDR) Gram-negative bacterial infections. As a cyclic lipopeptide, it shares the core mechanism of action with traditional polymyxins, such as polymyxin B and colistin, by binding to the lipid A component of lipopolysaccharides (LPS) in the bacterial outer membrane. This binding leads to the displacement of divalent cations, membrane disruption, and subsequent cell death. CB-182,804 was specifically engineered using a proprietary medicinal chemistry platform to enhance therapeutic efficacy while significantly reducing nephrotoxicity, which is the primary dose-limiting toxicity associated with older polymyxin antibiotics. It has demonstrated potent preclinical activity against a broad spectrum of "ESKAPE" pathogens, including carbapenem-resistant *Enterobacteriaceae*, *Pseudomonas aeruginosa*, and *Acinetobacter baumannii*.
