Overview

CBM588 + nivolumab + ipilimumab is a combination therapy investigated primarily for advanced or metastatic renal cell carcinoma and other cancers. CBM588 is a bifidogenic live bacterial product consisting of Clostridium butyricum MIYAIRI 588, designed to modulate and enrich the gut microbiome and enhance host immune responses. Nivolumab is a monoclonal antibody against programmed cell death protein 1 (PD-1) that blocks inhibitory signaling on T cells, and ipilimumab is a monoclonal antibody targeting cytotoxic T-lymphocyte-associated protein 4 (CTLA-4), also resulting in T cell activation. The combination aims to synergistically augment immune checkpoint inhibition and immunomodulation, leveraging both gut microbiota-derived immune stimulation and checkpoint blockade, to improve anti-tumor responses[1][3][5]. Preliminary studies show that CBM588 may increase the clinical efficacy of nivolumab and ipilimumab in renal cell carcinoma, possibly by increasing beneficial gut bacteria, immune-stimulatory cytokines, and favorably altering regulatory T-cell dynamics[1][3][5].
