CCL21-DC + pembrolizumab is a combination immunotherapy approach for advanced non-small cell lung cancer (NSCLC). It combines intratumoral injection of autologous dendritic cells genetically modified to express the chemokine CCL21 (CCL21-DC) with intravenous administration of pembrolizumab (anti-PD-1). CCL21 promotes co-localization of naive T cells and antigen-experienced dendritic cells to facilitate T cell activation, converting the tumor site into a lymph node-like environment. This in situ vaccination approach takes advantage of the full repertoire of tumor antigens to promote both local and systemic T cell activation. The addition of pembrolizumab aims to overcome the PD-L1 upregulation observed after CCL21-DC monotherapy, potentially enhancing antitumor immune responses in patients who have progressed on prior therapies.
