Overview

CD133+ cells (PETHEMA Foundation) is an autologous cell therapy being investigated for the treatment of critical limb ischemia (CLI), particularly in diabetic patients. The therapy involves the mobilization of CD133-positive hematopoietic stem and progenitor cells from the patient's bone marrow into the peripheral blood using Granulocyte Colony-Stimulating Factor (G-CSF). These cells are subsequently collected via apheresis, enriched using immunomagnetic selection, and administered through multiple intramuscular injections into the ischemic limb. The mechanism of action is based on therapeutic angiogenesis, where the transplanted CD133+ cells promote the formation of new microvasculature and improve tissue perfusion through the secretion of pro-angiogenic growth factors (paracrine effect) and potential direct differentiation into endothelial cells. This approach aims to provide a revascularization option for patients with 'no-option' CLI who are at high risk of amputation.
