CD19 CAR-T (4-1BB) refers to a chimeric antigen receptor (CAR) T-cell therapy platform that targets the B-lymphocyte antigen CD19 and utilizes a 4-1BB (CD137) costimulatory signaling domain. This architecture is designed to treat various B-cell malignancies, including mantle cell lymphoma (MCL), diffuse large B-cell lymphoma (DLBCL), and acute lymphoblastic leukemia (ALL). The 4-1BB costimulatory domain is associated with enhanced metabolic fitness and longer in vivo persistence of the CAR-T cells compared to CD28-based constructs, which often results in a more gradual expansion and a potentially lower risk of severe cytokine release syndrome (CRS) and neurotoxicity. The specific construct described in the context was developed and manufactured by the Institute of Hematology at Zhejiang University and has demonstrated durable complete remissions in patients with relapsed or refractory mantle cell lymphoma.
