DUALGRAFT is an investigational cellular therapy consisting of an engineered hematopoietic stem cell transplant (HSCT) graft designed to induce immune tolerance in solid organ transplant (SOT) recipients. Developed by Francisco Hernández Oliveros, the therapy involves a multi-step graft engineering process: the graft is enriched for CD34+ hematopoietic progenitor cells to ensure robust engraftment and depleted of naïve T-lymphocytes (CD45RA+) to minimize the risk of graft-versus-host disease (GvHD) and acute rejection. To maintain protective immunity and facilitate the induction of mixed chimerism, the graft is supplemented with donor memory T-lymphocytes (CD45RO+), which provide anti-viral and anti-pathogen protection without the high alloreactive potential associated with naïve cells. In cases involving haploidentical donors, the protocol may also include the infusion of donor-derived NK cells (CD56+). The primary therapeutic goal is to establish stable mixed hematopoietic chimerism, potentially allowing for the reduction or complete withdrawal of lifelong immunosuppressive therapy in patients undergoing kidney, lung, or multivisceral transplantation.
