CD34+ enriched peripheral blood stem cells with mononuclear cell addback is an investigational cellular therapy designed for allogeneic hematopoietic stem cell transplantation (alloSCT). The process involves mobilizing peripheral blood stem cells (PBSCs) using G-CSF, followed by positive selection for CD34+ cells to deplete T-cells and reduce the risk of acute graft-versus-host disease (aGVHD). To mitigate the risks of delayed immune reconstitution and opportunistic infections associated with pure CD34+ selection, a fixed, low dose of peripheral blood mononuclear cells (PBMNCs) — specifically 2×10⁵ CD3+ T cells/kg — is added back to the graft. This approach aims to balance the prevention of GVHD with the promotion of rapid engraftment and robust immune recovery. It has been evaluated in pediatric and adult patients with various hematologic malignancies and non-malignant conditions like sickle cell disease.
