CD34+ selected stem cell boost (CD34+-SCB) is a cellular therapy procedure involving the enrichment and infusion of CD34+ hematopoietic stem and progenitor cells (HSPCs) from a donor's mobilized peripheral blood into a recipient, typically without prior conditioning. This intervention is primarily used to treat complications following allogeneic hematopoietic stem cell transplantation (allo-HSCT), such as poor graft function (PGF), prolonged isolated thrombocytopenia, or mixed chimerism. By providing a concentrated dose of progenitor cells while depleting T-cells (via CD34+ selection, often using systems like CliniMACS), the boost aims to restore hematopoiesis and improve donor chimerism with a reduced risk of inducing or exacerbating graft-versus-host disease (GVHD). It is also being explored for persistent cytopenias following CAR T-cell therapy.
