Overview

CD4+ T-cell-depleted haploidentical hematopoietic transplant is an investigational cell therapy protocol being evaluated for the treatment of severe sickle cell disease. Developed by City of Hope, this approach utilizes a hematopoietic stem cell graft from a half-matched (haploidentical) related donor. The graft undergoes selective depletion of CD4+ T cells to reduce the risk of graft-versus-host disease (GvHD) while preserving other immune cells that facilitate engraftment and provide protection against infections. This transplant is typically administered following a non-myeloablative conditioning regimen, such as COH-MC-17, which aims to achieve stable mixed chimerism—a state where donor and recipient blood cells coexist—thereby allowing for the production of healthy red blood cells with lower toxicity than traditional full-match myeloablative transplants.
