Overview

CD4-CAR2 + maC46 is an investigational cell therapy that combines autologous cytomegalovirus (CMV)-specific T cells engineered with two genetic modifications: (1) CD4-CAR2—a chimeric antigen receptor (CAR) that expresses the extracellular domain of CD4 fused to CD28 and TCRζ (CD3-zeta) intracellular signaling domains, redirecting cytotoxic T lymphocyte (CTL) activity specifically against the HIV envelope glycoprotein—and (2) maC46, a membrane-anchored fusion inhibitor that protects the modified cells from HIV/SHIV infection. This dual-modified product is intended as an adoptive T cell immunotherapy to target and eliminate HIV-infected cells, with the additional goal of ensuring persistence and protection of the engineered T cells in vivo. The therapy is under preclinical and clinical evaluation for HIV infection and is developed in the context of ART interruption scenarios, aiming to control or delay viral rebound through immune-based clearance[1][4].
