CD45RA depleted leukocytes are an investigational cellular therapy product used in the context of allogeneic hematopoietic stem cell transplantation (HSCT) for non-malignant hematologic disorders. The product is prepared by the selective removal of naive T cells, which express the CD45RA antigen, from a donor's leukocyte or peripheral blood stem cell graft. This depletion is typically achieved using the CliniMACS system. By removing naive T cells—the primary mediators of graft-versus-host disease (GvHD)—while preserving memory T cells (CD45RO+), the therapy aims to reduce the risk of severe GvHD while maintaining the donor's ability to provide rapid immune reconstitution and protection against viral infections. This approach is being pioneered by Paul Szabolcs at UPMC Children's Hospital of Pittsburgh for pediatric and adult patients with severe sickle cell disease, beta-thalassemia major, and Diamond-Blackfan anemia who require alternative donor transplants.
