CD8 Depleted Donor Lymphocyte Infusion is an investigational cellular immunotherapy developed at the M.D. Anderson Cancer Center. It is designed to treat patients with relapsed hematologic malignancies following allogeneic hematopoietic stem cell transplantation. The therapy involves collecting peripheral blood lymphocytes from the original transplant donor and selectively depleting CD8+ cytotoxic T cells using immunomagnetic techniques, while retaining CD4+ helper T cells. The rationale behind this approach is that CD8+ T cells are primary mediators of graft-versus-host disease (GVHD), whereas CD4+ T cells can provide a potent graft-versus-leukemia (GVL) effect. By selectively removing CD8+ cells, the therapy aims to induce disease remission with a lower risk of severe acute GVHD compared to unmanipulated donor lymphocyte infusions.
