Cediranib + irinotecan is an investigational combination therapy consisting of cediranib, a highly potent oral inhibitor of all three vascular endothelial growth factor (VEGF) receptors, and irinotecan, a topoisomerase I inhibitor used as chemotherapy. Cediranib acts by blocking VEGF-mediated angiogenesis in tumors, thereby inhibiting tumor blood vessel formation and growth. Irinotecan interferes with DNA replication in cancer cells by inhibiting the enzyme topoisomerase I, leading to cell death. The combination has been studied for synergistic anti-tumor effects in various advanced solid tumors and metastatic cancers. Preclinical studies have shown that combining cediranib with cytotoxic agents like irinotecan can result in significantly enhanced tumor growth inhibition compared to monotherapy[4]. Clinical trials have evaluated the safety and pharmacokinetics of this regimen; it was generally well tolerated without major pharmacokinetic interactions between the two drugs[1][3]. Objective responses were observed even in patients who had previously progressed on similar regimens without cediranib[1].
