"R-National Cancer Institute (NCI)-brain metastasis" refers to a therapeutic regimen consisting of the small molecule tyrosine kinase inhibitor cediranib (AZD2171) administered in combination with whole-brain radiation therapy (WBRT). Cediranib acts as a potent inhibitor of vascular endothelial growth factor receptors (VEGFR-1, -2, and -3), as well as platelet-derived growth factor receptors (PDGFR) and c-Kit, thereby exerting anti-angiogenic and anti-tumor effects. This specific combination was evaluated in Phase 1 clinical trials sponsored by the National Cancer Institute (NCI) for the treatment of patients with brain metastases, particularly those originating from non-small cell lung cancer (NSCLC) and other solid tumors. The rationale for the combination is that cediranib may enhance the efficacy of radiation therapy by normalizing tumor vasculature and reducing peritumoral edema, potentially improving local control of metastatic lesions in the brain.
