Cefamandole + tobramycin is a combination of two antibiotics used primarily for the treatment of severe infections caused by susceptible Gram-negative and some Gram-positive bacteria. Cefamandole is a second-generation cephalosporin (beta-lactam) antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, leading to cell lysis. Tobramycin is an aminoglycoside antibiotic that binds to the bacterial 30S and 50S ribosomal subunits, preventing formation of the 70S initiation complex and thereby inhibiting protein synthesis, resulting in bactericidal activity. The combination has been shown to act synergistically against certain pathogens and is used in clinical settings for serious infections such as septicemia, lower respiratory tract infections, urinary tract infections, peritonitis, skin/soft tissue infections, and bone/joint infections. This regimen may be associated with nephrotoxicity but can be effective when other options are limited[1][4][5][8].
