Cefazolin + aztreonam is a dual-antibiotic regimen combining cefazolin, a first-generation cephalosporin, with aztreonam, a monobactam beta-lactam antibiotic. Cefazolin acts by inhibiting bacterial cell wall synthesis through binding to penicillin-binding proteins (PBPs), primarily targeting Gram-positive organisms such as *Staphylococcus aureus* (excluding MRSA) and some Streptococci. Aztreonam selectively binds to PBP-3 in susceptible aerobic Gram-negative bacteria—including Enterobacteriaceae—providing complementary spectrum without significant cross-reactivity in patients allergic to other beta-lactams except ceftazidime/ceftolozane[6]. This combination has been studied as empirical therapy for febrile episodes in cancer patients with adequate neutrophil counts.
