COSBACT is an oral fixed-dose combination antibiotic developed by Cospex Pharmaceuticals, primarily consisting of the third-generation cephalosporin cefixime and the beta-lactamase inhibitor sulbactam. Cefixime exerts its bactericidal activity by interfering with bacterial cell wall synthesis through the inhibition of penicillin-binding proteins (PBPs), which are essential for peptidoglycan cross-linking. Sulbactam acts as a suicide inhibitor of various beta-lactamases, thereby extending the spectrum of cefixime to include many beta-lactamase-producing strains that would otherwise be resistant. This combination is typically indicated for the treatment of susceptible bacterial infections, including upper and lower respiratory tract infections, urinary tract infections, and uncomplicated gonorrhea.
