Overview

Cefotaxime + ceftriaxone is a combination of two third-generation cephalosporin antibiotics, both of which are β-lactam antimicrobials. These agents act by inhibiting bacterial cell wall synthesis through binding to penicillin-binding proteins (PBPs), leading to bactericidal activity. Both drugs have broad-spectrum activity against Gram-positive and Gram-negative bacteria, and are commonly used in the treatment of severe infections such as meningitis, sepsis, respiratory tract infections, urinary tract infections, and others. While their antibacterial spectra overlap significantly, they differ in pharmacokinetic properties such as half-life (cefotaxime has a shorter half-life than ceftriaxone), protein binding (cefotaxime lower than ceftriaxone), metabolism (cefotaxime primarily hepatic; ceftriaxone intestinal), and elimination routes. The combination may be considered for synergistic effects or specific clinical scenarios but is not standard practice due to their similar mechanisms[5][8][1].
