Ceftazidime-avibactam + aztreonam is a combination antibiotic regimen being evaluated in the TREAT-GNB platform trial, led by the National University of Singapore, for the treatment of severe infections caused by carbapenem-resistant Gram-negative bacteria. This combination is particularly significant for treating pathogens that produce metallo-beta-lactamases (MBLs), such as NDM, VIM, or IMP. Ceftazidime is a third-generation cephalosporin that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). Avibactam is a non-beta-lactam beta-lactamase inhibitor that neutralizes Class A, Class C, and some Class D beta-lactamases. Aztreonam, a monobactam, is inherently stable against MBLs but is susceptible to other beta-lactamases often co-produced by MBL-carrying strains. In this combination, avibactam protects aztreonam from degradation by these co-produced enzymes, allowing aztreonam to exert its bactericidal effect against MBL-producing Enterobacterales.
