ceftolozane + avibactam is an investigational antimicrobial combination consisting of the fifth-generation cephalosporin ceftolozane and the non-beta-lactam beta-lactamase inhibitor avibactam. Ceftolozane exerts its bactericidal activity by binding to penicillin-binding proteins (PBPs), specifically PBP3, PBP1b, and PBP1c, which inhibits bacterial cell wall synthesis and leads to cell death. Avibactam is a diazabicyclooctane that covalently and reversibly inhibits a broad range of beta-lactamases, including Ambler Class A (such as KPC), Class C (AmpC), and certain Class D (such as OXA-48) enzymes. This combination is being evaluated, notably in clinical trials sponsored by Assistance Publique Hopitaux De Paris, for the treatment of severe gram-negative infections such as sepsis and hospital-acquired sepsis. By pairing ceftolozane's high stability against Pseudomonas-specific resistance mechanisms with avibactam's broad-spectrum enzyme inhibition, the regimen aims to overcome resistance in multi-drug resistant (MDR) *Pseudomonas aeruginosa* and carbapenem-resistant Enterobacteriaceae.
