Overview

Ceftriaxone + gentamicin is a combination of two antibiotics used primarily for the treatment of severe bacterial infections, including endocarditis and certain resistant or complicated infections. Ceftriaxone is a third-generation cephalosporin that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, leading to cell lysis and death. Gentamicin is an aminoglycoside antibiotic that binds to the 30S ribosomal subunit, inhibiting protein synthesis and causing misreading of mRNA in susceptible bacteria. The combination can provide synergistic bactericidal activity against certain pathogens, particularly in serious infections like endocarditis caused by methicillin-susceptible Staphylococcus aureus (MSSA) or viridans group streptococci. This regimen may be considered for outpatient therapy after stabilization in selected patients but requires careful monitoring due to increased risk of nephrotoxicity[1][5][3].
