This antimicrobial protocol, investigated by the University of Duhok, combines systemic and local antibiotic prophylaxis to mitigate the risk of deep infections following anterior cruciate ligament (ACL) reconstruction. The regimen utilizes parenteral ceftriaxone, a third-generation cephalosporin, alongside a vancomycin-soaked autograft. Ceftriaxone is administered intravenously at induction and postoperatively to provide systemic coverage. Vancomycin, a glycopeptide antibiotic, is applied locally by soaking the tendon graft in a solution for 20 minutes prior to implantation. This dual-mechanism approach targets a broad spectrum of pathogens, specifically addressing the high incidence of Gram-positive infections, such as *Staphylococcus aureus*, which are common causes of graft contamination and subsequent arthrofibrosis or articular cartilage degeneration in orthopedic procedures.
