Celecoxib + fluorouracil + leucovorin + oxaliplatin is a combination chemotherapy and targeted therapy regimen investigated for the adjuvant treatment of resected stage III colon cancer. The regimen adds celecoxib, a selective cyclooxygenase-2 (COX-2) inhibitor, to the standard-of-care FOLFOX chemotherapy backbone, which consists of the antimetabolite fluorouracil, the biochemical modulator leucovorin, and the platinum-based alkylating agent oxaliplatin. Celecoxib is intended to inhibit the COX-2 enzyme, which is frequently overexpressed in colorectal tumors and associated with inflammation-mediated tumor growth and proliferation. This specific combination was the focus of the large-scale Phase III CALGB/SWOG 80702 trial conducted by the Alliance for Clinical Trials in Oncology. While the primary analysis of the trial did not show a statistically significant improvement in disease-free survival for the overall population, the regimen remains a subject of research regarding potential benefits in specific molecular subgroups, such as those with PIK3CA mutations or detectable circulating tumor DNA (ctDNA).
