This is a combination of three small molecule drugs—celecoxib, methotrexate, and simvastatin—each with distinct mechanisms of action and primary indications. Celecoxib is a selective cyclooxygenase-2 (COX-2) inhibitor used primarily for pain and inflammation in conditions such as osteoarthritis and rheumatoid arthritis. Methotrexate is an antimetabolite and antifolate agent commonly used as a disease-modifying antirheumatic drug (DMARD) for autoimmune diseases like rheumatoid arthritis, psoriasis, and certain cancers. Simvastatin is an HMG-CoA reductase inhibitor (statin) indicated for lowering cholesterol to reduce cardiovascular risk. There are significant pharmacokinetic interactions among these agents: - Celecoxib can increase plasma concentrations of methotrexate by inhibiting its renal tubular secretion via the organic anion transporter 3 (OAT3), potentially increasing toxicity risk[1][2][5]. - Methotrexate combined with statins such as simvastatin increases the risk of hepatotoxicity[7][8]. - Simvastatin’s metabolism may be affected by other drugs but has no direct interaction with celecoxib reported[6]. The combination should only be considered under close medical supervision due to increased risks of nephrotoxicity, hepatotoxicity, myopathy/rhabdomyolysis, gastrointestinal bleeding/ulceration, cardiovascular events, and other adverse effects[1][3][7][8][9].
