Overview
celecoxib + pembrolizumab + gemcitabine + cisplatin is an investigational combination regimen being studied for the treatment of CK5/6-high unresectable locally advanced or metastatic intrahepatic cholangiocarcinoma (ICC). This four-drug regimen integrates the COX-2 inhibitor celecoxib, the PD-1 checkpoint inhibitor pembrolizumab, and the cytotoxic chemotherapy agents gemcitabine and cisplatin. The rationale for this combination is based on the hypothesis that COX-2 inhibition by celecoxib can modulate the tumor microenvironment to be more immunogenic, potentially synergizing with the immune-restoring effects of pembrolizumab and the DNA-damaging effects of gemcitabine and cisplatin. The regimen is currently being evaluated in a Phase Ib/II clinical trial sponsored by Shanghai Jiao Tong University Affiliated Sixth People's Hospital.
