**cemiplimab + carboplatin + pemetrexed** is a combination regimen used as first-line treatment for locally advanced or metastatic non-squamous non-small cell lung cancer (NSCLC) in patients not suitable for curative surgery or definitive chemoradiation. Cemiplimab is a monoclonal antibody that blocks the programmed cell death protein 1 (PD-1), restoring T-cell mediated immune response against cancer cells. Carboplatin is a small molecule platinum-based chemotherapy agent causing DNA crosslinking and apoptosis, and pemetrexed is a small molecule antifolate that inhibits folate-dependent enzymes in purine and pyrimidine synthesis, leading to cell death. This regimen improves survival and is generally well tolerated, as shown in randomized phase III trials.
