Overview

Cemiplimab + fianlimab is a combination of two fully human monoclonal antibodies developed for cancer immunotherapy. Cemiplimab targets the immune checkpoint receptor Programmed cell death protein 1 (PD-1) on T cells, blocking its interaction with PD-L1 and PD-L2 to enhance anti-tumor immune responses. Fianlimab targets the Lymphocyte activation gene 3 protein (LAG-3) receptor on T cells, inhibiting LAG-3/MHC class II–driven T-cell inhibition. The rationale for combining these agents is that dual blockade of PD-1 and LAG-3 can overcome resistance to single-agent checkpoint inhibitors and further release inhibitory brakes on T cell activation. This combination is being investigated primarily in advanced melanoma, where it has shown promising response rates in clinical trials[2][5][6][8].
