Overview

CEOP + IVE + GDP is an alternating multi-drug chemotherapy regimen used in the treatment of newly diagnosed peripheral T cell lymphoma (PTCL), excluding anaplastic large cell lymphoma-anaplastic lymphoma kinase positive. The regimen alternates three different combinations over six cycles, each administered every 21 days: - **CEOP** (Cyclophosphamide, Epirubicin, Vincristine, Prednisone): Administered in cycles 1 and 4. - **IVE** (Ifosfamide, Epirubicin, Etoposide): Administered in cycles 2 and 5. - **GDP** (Gemcitabine, Cisplatin, Dexamethasone): Administered in cycles 3 and 6. Each component targets rapidly dividing cancer cells through distinct mechanisms: - Alkylating agents (cyclophosphamide, ifosfamide) crosslink DNA, - Anthracyclines (epirubicin) intercalate DNA and inhibit topoisomerase II, - Vinca alkaloids (vincristine) disrupt microtubule formation, - Corticosteroids (prednisone, dexamethasone) induce apoptosis in lymphoid cells, - Platinum compounds (cisplatin) form DNA adducts causing apoptosis, - Nucleoside analogues (gemcitabine) inhibit DNA synthesis, - Topoisomerase II inhibitors (etoposide) prevent DNA repair. This combination was evaluated for efficacy and safety compared to standard CEOP alone as first-line therapy for PTCL[1][2][3].
