Ceralasertib + durvalumab is an investigational combination therapy for cancer. Ceralasertib is a selective inhibitor of Ataxia Telangiectasia and Rad3-related (ATR) protein kinase, which plays a key role in the DNA damage response; by inhibiting ATR, ceralasertib enhances DNA damage in tumor cells and induces apoptosis. It also modulates immune and inflammatory pathways[1][6][8]. Durvalumab is a high-affinity human IgG1 monoclonal antibody that targets programmed death-ligand 1 (PD-L1), blocking its interaction with PD-1 on T-cells to reactivate anti-tumor immunity[2][5][6]. The combination aims to synergistically enhance anti-tumor immune responses by increasing tumor immunogenicity through DNA damage while simultaneously releasing immune inhibition via PD-L1 blockade. This regimen has been studied in multiple cancers including non-small cell lung cancer (NSCLC), small cell lung cancer (SCLC), advanced gastric cancer, and melanoma resistant to prior immunotherapy[3][4][5].
