Cerebrolysin + donepezil is a combination therapy used primarily in the treatment of Alzheimer's disease. Cerebrolysin is a neuropeptide preparation with neurotrophic and neuroprotective properties, believed to act by inhibiting glycogen synthase kinase 3 beta (GSK3β) and activating signaling pathways such as PI3K/Akt, which may enhance brain-derived neurotrophic factor (BDNF) expression. Donepezil is a selective and reversible acetylcholinesterase inhibitor that increases acetylcholine levels in the brain, thereby improving cholinergic neurotransmission. The combination has shown synergistic effects in increasing serum BDNF levels and improving cognitive outcomes compared to either agent alone, particularly in patients with the apolipoprotein E epsilon-4 allele. Clinical studies indicate that this combination can delay cognitive decline and improve global functioning in mild-to-moderate Alzheimer's disease[1][2][5].
