Certolizumab pegol + cisplatin + pemetrexed is a combination therapy investigated by Memorial Sloan Kettering Cancer Center for the treatment of stage IV lung adenocarcinoma. Certolizumab pegol is a PEGylated, humanized antigen-binding fragment (Fab') of a TNF-alpha monoclonal antibody that neutralizes the pro-inflammatory cytokine TNF-alpha. Cisplatin is a platinum-based chemotherapy agent that cross-links DNA, while pemetrexed is a folate antimetabolite that inhibits multiple enzymes involved in pyrimidine and purine synthesis. The therapeutic rationale for this combination is based on the observation that chemotherapy can induce a TNF-alpha-mediated inflammatory loop that promotes tumor survival and metastasis; by adding certolizumab pegol, researchers aim to disrupt this loop and enhance the efficacy of the standard-of-care cisplatin/pemetrexed regimen in advanced lung cancer patients.
