Cetuximab + cixutumumab + irinotecan is an investigational combination therapy composed of three agents: - Cetuximab, a chimeric monoclonal antibody targeting the epidermal growth factor receptor (EGFR), inhibits EGFR-mediated signaling pathways involved in tumor cell proliferation and survival. - Cixutumumab, a fully human monoclonal antibody directed against the insulin-like growth factor 1 receptor (IGF-1R), blocks IGF-1R signaling implicated in cancer cell growth and resistance mechanisms. - Irinotecan, a small molecule topoisomerase I inhibitor, interferes with DNA replication in rapidly dividing cells leading to cytotoxicity. This combination is being explored primarily for advanced or metastatic colorectal cancer that has become refractory to standard therapies. The rationale for combining these agents is to simultaneously inhibit multiple pathways involved in tumor progression and drug resistance. While cetuximab plus irinotecan is an established regimen for irinotecan-refractory metastatic colorectal cancer[1][2][3][4][5], the addition of cixutumumab aims to further suppress compensatory survival signals mediated by IGF-1R.
