Cetuximab + irinotecan + simvastatin is a combination regimen investigated primarily for the treatment of metastatic colorectal cancer (mCRC), especially in patients with KRAS mutations who have failed prior standard therapies. Cetuximab is a monoclonal antibody targeting the epidermal growth factor receptor (EGFR), inhibiting downstream signaling pathways involved in tumor cell proliferation and survival. Irinotecan is a topoisomerase I inhibitor that interferes with DNA replication, leading to cell death. Simvastatin, an HMG-CoA reductase inhibitor (statin), blocks the mevalonate pathway and has been hypothesized to interfere with post-translational modification of KRAS, potentially restoring sensitivity to EGFR inhibition in KRAS-mutant tumors. Preclinical studies suggested synergistic effects between these agents; however, clinical trials did not demonstrate significant restoration of cetuximab sensitivity in KRAS-mutant colorectal cancer patients[1][2][3][5]. The combination has shown some efficacy and safety signals but remains investigational.
