Cetuximab + lenalidomide is a combination therapy investigated primarily for advanced solid tumors, including metastatic colorectal cancer and head and neck squamous cell carcinoma. Cetuximab is a monoclonal antibody that targets the epidermal growth factor receptor (EGFR), blocking ligand binding and downstream signaling pathways involved in tumor cell proliferation, survival, motility, and invasion. Lenalidomide is an immunomodulatory small molecule that alters cytokine production, enhances T-cell co-stimulation, increases natural killer (NK) cell-mediated cytotoxicity, and modulates substrate specificity of the cullin ring E3 ubiquitin ligase complex via cereblon binding. This leads to degradation of key transcription factors essential for malignant B-cell survival. The combination has been studied in phase I/II trials; while it was generally well tolerated with expected toxicities (notably increased thromboembolic risk), it did not demonstrate clinically meaningful efficacy in KRAS-mutant metastatic colorectal cancer[1][2][3][4][5].
