The toxicity profile is relatively favorable compared to conventional regimens like M-VAC or CMV. When cetuximab is added to chemotherapy regimens for colorectal cancer, it has been shown to improve progression-free survival, overall survival, and objective response rates compared to chemotherapy alone in patients with KRAS wild-type tumors[1]. ## Administration The P-HDFL regimen typically involves: - Cisplatin 35 mg/m² given as a 24-hour infusion - High-dose 5-FU 2,600 mg/m² as a 24-hour infusion - Leucovorin 300 mg/m² as a 24-hour infusion These are administered on Days 1 and 8 of a 4-week cycle, with only HDFL (5-FU and leucovorin) given again on Day 15[8]. Cetuximab is typically administered at an initial dose of 400 mg/m², followed by weekly doses of 250 mg/m²[4]. ## Mechanism of Action The combination works through complementary mechanisms: 1. **Cetuximab**: Inhibits EGFR signaling, leading to decreased cell proliferation, survival, and angiogenesis[5]. 2. **Cisplatin**: Forms DNA crosslinks, interfering with cell division. 3. **5-FU**: Inhibits thymidylate synthase and incorporates into DNA and RNA, disrupting nucleic acid function[9]. 4. **Leucovorin**: Enhances the cytotoxic effects of 5-FU by stabilizing the binding of 5-FU metabolites to thymidylate synthase.
