Cetuximab beta + fruquintinib is an investigational combination therapy being evaluated for the first-line treatment of pMMR, RAS/BRAF wild-type unresectable metastatic colorectal cancer (mCRC). This regimen combines cetuximab beta, a chimeric monoclonal antibody targeting the epidermal growth factor receptor (EGFR), with fruquintinib, a highly selective small-molecule tyrosine kinase inhibitor of vascular endothelial growth factor receptors (VEGFR-1, -2, and -3). By simultaneously inhibiting the EGFR pathway, which drives tumor cell proliferation, and the VEGF pathway, which promotes angiogenesis, the combination aims to provide a chemotherapy-free alternative for patients who may not tolerate or prefer to avoid cytotoxic drugs. The clinical trial sponsored by Zhejiang University also explores the addition of immune checkpoint inhibitors, such as sintilimab, to this dual-targeted backbone to potentially enhance antitumor activity.
