Overview

This combination therapy involves administering gonadotropin-releasing hormone (GnRH) analogs concurrently with chemotherapy, primarily to protect ovarian function in premenopausal women. GnRH analogs work by suppressing the gonadotropin levels to simulate pre-pubertal hormonal milieu, preventing primordial follicles from maturation and decreasing the number of follicles vulnerable to chemotherapy-induced damage. This approach has shown effectiveness in reducing the risk of premature ovarian insufficiency (POI) and preserving fertility, particularly in women with breast cancer and some hematological malignancies. The most common GnRH analogs used include agonists like Goserelin, Buserelin, and Leuprolide, as well as GnRH antagonists.
