Chidamide + abemaciclib + endocrine therapy is a combination regimen being evaluated for the treatment of hormone receptor-positive (HR+), human epidermal growth factor receptor 2-negative (HER2-) metastatic breast cancer. This regimen is specifically targeted at patients who have previously progressed on a CDK4/6 inhibitor such as palbociclib. Chidamide is a selective histone deacetylase (HDAC) inhibitor that targets HDAC1, 2, 3, and 10, aiming to reverse epigenetic changes that contribute to drug resistance. Abemaciclib is a CDK4/6 inhibitor that prevents cell cycle progression from the G1 to S phase. The addition of endocrine therapy (such as fulvestrant or aromatase inhibitors) provides continued suppression of the estrogen receptor pathway. The combination is being studied in a Phase Ib/II trial led by Dr. Biyun Wang at Fudan University Shanghai Cancer Center.
