Chlorambucil + methylprednisolone + prednisone is a combination regimen of three drugs: chlorambucil, a nitrogen mustard alkylating agent used as an antineoplastic (anti-cancer) and immunosuppressant; methylprednisolone, a synthetic glucocorticoid corticosteroid with potent anti-inflammatory and immunosuppressive activity; and prednisone, an oral corticosteroid with predominant glucocorticoid action. Chlorambucil acts by cross-linking DNA, thus interfering with cancer cell replication and antibody production, and is primarily indicated for the treatment of chronic lymphocytic leukemia (CLL) and lymphomas. Methylprednisolone and prednisone modulate inflammation and immune responses by binding the glucocorticoid receptor and altering gene expression, and are frequently used in various autoimmune, inflammatory, and hematological conditions. This specific three-drug combination is not a standard or widely recognized regimen but all three drugs, especially chlorambucil plus corticosteroids (prednisone or methylprednisolone), have been studied in combination for certain hematologic cancers and nephrotic syndrome. Each drug is generally administered orally and dosing regimens vary depending on indication and patient characteristics. There is evidence that combinations of alkylating agents with corticosteroids can increase response rates and remission duration in CLL and idiopathic nephrotic syndrome[1][3][5][9].
