It is used primarily for deep sedation and as an adjunct in anesthesia, especially in neurosurgical or critical care settings. The combination leverages the pharmacological actions of each component: - **Chlorpromazine** is a phenothiazine antipsychotic that acts mainly as a dopamine receptor antagonist with additional antihistaminic and anticholinergic effects. It provides strong sedative and antiemetic properties. - **Promethazine** is a first-generation antihistamine (H1 receptor antagonist) with significant sedative, antiemetic, and anticholinergic effects. - **Pethidine** (also known as meperidine) is an opioid analgesic acting primarily on the mu-opioid receptor to provide pain relief. The mixture produces synergistic sedation, analgesia, muscle relaxation, hypothermia induction (for therapeutic hypothermia), reduction of cerebral metabolic rate/oxygen consumption, protection against brain edema after trauma or surgery, and prevention of secondary brain injury. This allows for lower doses of each drug than if used alone[1][3][4][6].
