Cholecalciferol + calcium carbonate is a combination nutritional supplement regimen investigated by the Medical University of Vienna for the prevention of bone loss in patients undergoing bariatric surgery. The intervention, evaluated in the BABS (Bariatric Surgery) trial, consists of high-dose cholecalciferol (Vitamin D3) and calcium supplementation to mitigate the rapid loss of bone mineral density (BMD) and adverse changes in bone metabolism markers that typically follow procedures such as laparoscopic Roux-en-Y gastric bypass and sleeve gastrectomy. Cholecalciferol acts as an agonist at the Vitamin D receptor (VDR), which enhances intestinal calcium absorption and regulates bone remodeling, while calcium carbonate provides the essential mineral substrate for bone mineralization. The regimen specifically utilized a pre-surgery loading dose of cholecalciferol followed by a maintenance dose and daily calcium intake post-surgery.
