Choline geranate + ketoconazole is an experimental topical antifungal formulation developed by the Shanghai Skin Disease Hospital. This combination utilizes choline geranate (CAGE), a deep eutectic solvent and ionic liquid, as a specialized vehicle to enhance the transdermal delivery of ketoconazole. Ketoconazole is a well-established imidazole antifungal that inhibits the enzyme lanosterol 14-alpha-demethylase, a critical step in the synthesis of ergosterol for fungal cell membranes. The CAGE component serves a dual purpose: it significantly increases the solubility and skin penetration of the hydrophobic ketoconazole by disrupting the lipid bilayer of the stratum corneum, and it possesses intrinsic antimicrobial properties that may act synergistically with the antifungal agent. The formulation is specifically designed to treat superficial fungal infections that are often resistant to standard topical treatments due to poor drug penetration.
