The combination of ciclosporin (also known as cyclosporine A or CsA) and mycophenolate mofetil (MMF) is used as an immunosuppressive regimen primarily to prevent organ transplant rejection and to treat certain autoimmune conditions. Ciclosporin is a calcineurin inhibitor that suppresses T-cell activation by inhibiting the transcription of interleukin-2 and other cytokines. Mycophenolate mofetil is a prodrug of mycophenolic acid that acts as a reversible inhibitor of inosine monophosphate dehydrogenase (IMPDH), leading to selective inhibition of lymphocyte proliferation by blocking de novo guanosine nucleotide synthesis. The combination has synergistic immunosuppressive effects and is commonly used in kidney, heart, and liver transplantation regimens[1][2][5]. It may also be considered for steroid- or calcineurin-inhibitor-resistant nephrotic syndrome such as focal segmental glomerulosclerosis[5].
